A Case of Paradoxical Convergence during Lateral Gaze

緒　言

　Duane症候群が異常神経支配によることは筋電図検査が眼科領域に応用されるようになつて初めて明らかになつた事実である^1〜3）。その後，外眼筋の異常神経支配にはDuane症候群ばかりでなく種々のものがあることがわかつてきた。Papstら^4〜6），井上・久保田^7），Huberら^8），丸尾・久保田^9），本村ら^10）の報告がみられる。最近，著者らは先天性に側方視（水平むき運動）に際して輻輳（内よせ）がおこるという未だかつて経験したことのないきわめてまれな眼球運動を示す症例を経験した。側方視に際して開散（外よせ）のおこる症例^11〜13）や側方視に際して輻輳様動運がおこる症例^13〜16）についての報告はあるが，ここにみるような症例はきわめて珍しいのでここに報告しておきたいと思う。

This paper describes congenital ocular move-ment disorder showing paradoxical converge-nce, miosis and accommodation during lateral gaze in a 23-year-old female. Manifest conver-gence could be elicited by an effort to lateral gaze in either direction. Corresponding findings could be documented by electrooculogram, elec-tromyogram, electropupillogram and objective accommodometry. Misdirected innervations to the external and inner ocular muscles of congenital nature seemed to be involved in this extremely rare condition.
