鼻涙管通過障害の原因としての顔面神経麻痺の意義について（症例考察）

Aetiologic significance of facial nerve palsy in the development of obstructio ductus nasolacrimalis

緒言

　慢性涙嚢炎は，鼻涙管通過障害（狭窄乃至閉塞）に続いておこることが多く，この通過障害が多くは結膜疾患から下行性に，或は，鼻腔副鼻腔疾患から上行性に，炎症の波及するに由ると，一般に既に認められているところであるが，結膜に炎症を有するもの，或は，鼻腔疾患を有するものゝすべてが，涙道疾患をおこしてくるとは言えないであろう。従つて，こゝに当然，何らかの発病素因が考えられる。従来，曰く遣伝的関係，曰く骨壁の構造の個人差，曰く涙分泌過剰等々^1），種々の問題がとりあげられて来た。涙道疾患は必ずしも単一な素因のもとに発病するものではないであろうが，私^2）は，涙嚢機能の個人差としての，その弱小も一素因をなしているのではないかと推察した。このことに関連して，次に一症例を挙げ，鼻涙管通過障害の原因としての顔面神経麻痺の持つ意義について考察を加えたい。

The author reported a case in which Obstructio ductus nasolacrimalis consecutively appeared after suffering from facial nerve palsy.

It is suggested that the functional disturbance of lacrimal pathway caused by facial nerve palsy plays an important role in the development of organic lesion of nasolacrimal duct ; that is, the incomplete conduction of tears is apt to lead to oblstruction of nasolacrimal duct, as if the ditch in which sewage flow slowly is frequently chocked with debris.
