外傷性硝子体前房内脱出の二例

Two cases of prolapsus corporis vitrei into the anterior chamber caused by trauma

　硝子体前房内脱出を細隙燈顕微鏡で最初に観察報告したのはErggeletで有り，Hesseは之を詳細に観察記載した。我国でも中村，諌山，加藤，豊島，田野辺，樋田氏等の他数氏の症例報告はあるが其の数は比較的少い。本症の発生機転には諸説があるが，水晶体とZinn氏帯が硝子体前面と眼房との間の障壁を成しているので，硝子体の前房への脱出には此の障壁の全体又は一部分の欠損によつて脱出路が形成されることが前提となり，更に外力の加わつた場合に起つて来るので手術，打撲等の外力の加わる事によつて発生する事が多い。我国の報告例も殆んどが打撲に原因している。硝子体前房内脱出は其の臨床所見より硝子体前境界層が破れずに硝子体を包んだまま脱出した場合は脱出部が嚢状又は茸状を呈し，前境界層が断裂を起した時は絮状を呈するとHaitz （1930年）は述べている。我々は最近定型的な嚢状を呈する症例及び硝子体が累状を呈し角膜後面に附着した症例を経験したので報告する。

The 1st case. Man of 31 years old. Right eye was injured during choping wood and the region, from the cornea to the ciliary body, was affected. Then occurred prolapsus corporis vitrei into the anterior chamber from an area of the pupil. Pigments from the iris and hemorrhage from the ciliary body have been accumulating in the basis of the cyst. The 2nd case. Woman of 31 years old.
