頭部外傷により内側縦束症候群（Syndrome of the Medial Longitudinal Fasciculus）を呈した1例

A Case of Syndrome of the Medial Longitudinal Fasciculus following Head Injury

緒言

　内側縦束症候群（Syndrome of the MedialLongitudinal Fasciculus，以下MLF症候群と略す）とは，脳幹の内側縦束の障害によつて惹起されるもので，臨床症状としては，眼球の水平性共同運動障害で，一般に輻較は正常であるにもかかわらず障害側眼球内転障害と他眼の外転眼に見られる水平性眼振を特徴とするものである。病因としては，脳血管障害，多発性硬化症，脳腫瘍，外傷，糖尿病，膠原病（SLE）脳炎，梅毒等があり，脳血管障害（特に片側性），多発性硬化症（特に両側性）によるものが多く，外傷によるものは，現在までのところBielschowsky^1），Jaensch^2），田中ら^3）の計3例の報告があるのみである。

　これらのうち両側性MLF症候群を呈した症例は，Bielschowskyの1例のみである。今回，われわれは，頭部外傷により両側性MLF症候群を呈した症例を経験したので，ここに報告する。

The occurrence of syndrome of medial longi-tudinal fasciculus (MLF) following head injury is a rather rare event and only 3 cases have been forthcoming in literature. Further, bilateral involvement of MLF after head injury has been reported only once. This paper describes a rare case with bilateral MLF syndrome follo-wing blunt head injury.

A 39-year old male suffered head concussion during iceskating and remained unconscious for half an hour. Eleven hours after the injury, he started to develop left ptosis. The patient sought medical advice two day after the injury.Diplopia was present on looking either side.
