The occurrence of syndrome of medial longi-tudinal fasciculus (MLF) following head injury is a rather rare event and only 3 cases have been forthcoming in literature. Further, bilateral involvement of MLF after head injury has been reported only once. This paper describes a rare case with bilateral MLF syndrome follo-wing blunt head injury.

A 39-year old male suffered head concussion during iceskating and remained unconscious for half an hour. Eleven hours after the injury, he started to develop left ptosis. The patient sought medical advice two day after the injury.Diplopia was present on looking either side.
