急性出血性結膜炎にみられた角膜点状上皮下混濁について

Subepithelial Punctate Opacities of the Cornea after Hemorrhagic Conjunctivitis

緒言

　急性出血性結膜炎（Acute hemorrhagic con—junctivitis以下AHCと略）は流行性出血性結膜炎Epidemic hemorrhagic conjunctivitisとも呼ばれるが，ここでは前者を用いることにする。これはガーナ国アクラ市での1969年における大流行^1）以前には全く知られなかつた疾患である。患者からウイルスが分離され，起炎病原体であることが決定したのは，わが国で甲野らによつてなされた研究の結果である^2）。わが国をも含めた世界的の規模における大流行の疫学は同氏によつて詳述されている^3）。

　AHCには発病初期に上皮性角膜炎が高率に合併し，これが患者の自覚症である眼痛や異物感などの原因として考えられている。しかしアデノウイルス角結膜炎にみるような，急性結膜炎の症状が消失したのちに生ずる点状の上皮下混濁は，AHCには起こらないとされてきた。そしてこのことが両疾患の臨床的鑑別点の一つともされている。

In the majority of patients with acute hemor-rhagic conjunctivitis, corneal involvement ap-pears as fine punctate epithelial keratitis whichis thought to be the cause of pain or foreign body sensation complained in the acute stage of this disease. However, subepithelial opactities of the cornea after conjunctivitis have not been documented.

One or two small and round subepithelial opacities were observed in 4 of 12 patients of this disease. They appeared 6 to 31 days after the onset, when the epithelial lesions had com-pletely healed.
