いわゆるGarcin's syndrome （Multiple cranial neuropathy）をきたした頭蓋底骨髄炎の1剖検例

A Case of Garcin's Syndrome (Multiple Cranial Neuropathy) Caused by the Basal Osteomyelitis of the Cranium

緒言

　後部副鼻腔炎が頭蓋内合併症をきたすという報告は古くは多く見受けられたが，最近の化学療法の発展により非常に稀なものとなつた。しかし今後抗生物質耐性菌の発生とともに，再びその診断および治療上種々の問題が発生すると考えられる。

　われわれは最近いわゆるGarcin症候群（またはMultiple cranial neuropathy）を呈し死亡した興味ある一症例を経験し，剖検の結果原発巣が後部副鼻腔炎と考えられ，経過中非常にまれとされている慢性の頭蓋内合併症をきたした症例を得たので報告する。

A rare case of Garcin's syndrome (multiple cranial neuropathy) was reported.

The case, 55-year old man who had slight diabetes mellitus suffered from the left orbital apex syndrome after high fevers and severe headaches which had begun 3 months earlier.

The syndrome developed to the Garcin's syn-drome (multiple cranial neuropathy) and the patient died 5 months later.

Pathological examinations showed the basal meningitis, basal osteomyelitis of the cranium which was thought to be derived from the posterior sinusitis.
