北海道内に発生したSMON患者の眼症状

Ophthalmological Aspects of Subacute Myelo-Optico-Neuropathy (SMON)

I．緒言

　近年日本各地から，下痢，腹痛等の腹部症状を前駆とする脳脊髄炎（いわゆるSMON）の報告が相次いでなされ，本症の独立性に関して内科学，神経科学，病理学，微生物学等の立場より色々検討されている。本症には又眼症状を伴う事があるために，眼科学的立場からも検討が加えられつつある。北海道においては，昭和33年以来釧路市を中心として又昭和37年以降は室蘭市周辺にも本症が多発し，更にその他の地域にも散発的に発生を見ている。今回我々は，この内の105例について眼科的検査を行い，本症に伴う眼障害の病型について検討したので，ここに報告する。

One hundred five patients with established diagnosis of SMON (subacute myelo-optico-neu-ropathy, defined by Tsubaki & Toyokura, 1965) from Hokkaido district have been subjected to through ophthalmological examinations. Nine-teen cases (18%) had ocular manifestations of the syndrome which consisted of bilateral cent-ral scotoma in all but one cases, constricted visual field in chronic cases and pallor of the optic discs. There was a consistent parallelity between the ocular symptoms and the severity of systemic manifestations. The exacerbation in ocular symptoms was usually preceded by aggravation of systemic manifestations.
