隆鼻術を目的とした油性ペニシリン注射による網膜中心動脈栓塞の1例

Embolism of Central Retinal Artery due to Injection of Oil-Penicillin

I．緒言

　心臓病や血管病の患者の一眼が，突如として失明し，再び恢復しないことがあると言うことは，古くから知られていたが，その原因，所見等はGraefeにより初めて記載され，網膜中心動脈栓塞と名付けられた。

　その後，この疾患に関しては，多数の報告があるが，その原因については，内外共に心臓疾患，腎臓疾患或いは末梢血管の血栓などが主役をつとめて居り，原因不明なものもかなり多いと言われる。一方では，原因的関係は明らかであるが，極めて特殊なものとして，眼窩蜂窩織炎，鼻根部パラフィン注射等による網膜中心動脈栓塞が報告されている。

The 19 years old female injected oil procain Penicillin to the nasal base region by herself for the purpose of the rhinoplastic operation. Soon after starting injection, she suffered from nausea, numbness in the face and poor vision of the left eye.

Swelling of the left eye lid and conjunctival edema were seen. The Ophthalmoscopic exa-mination revealed severe embolia arteriae cen-tralis retinae having oil droplets in some of blood vessels, indicating the fat embolism of the central retinal artery.

Vision could not recoverd.
