油性ペニシリンの注射に因る網膜中心動脈栓塞

Embolism of central retinal artery due to oil-Penicillin

　著者の一人浦山は，さきに「骨折の際に見らるる眼底変化」と題して所謂Purtscher氏病の再吟味を行い，眼部脂肪栓塞に関する知見を発表した。この種の栓塞は必ずしも体内脂肪組織にのみ由来するとは限らず，体外性の脂肪性物質に因り起つても支えはないわけである。恰もその頃，注射薬としてのペニシリン油剤が国内に普及せられつつあつたので，それに因る障碍が或いは起りはしまいかと秘かに注意していたところ，他科領域では下肢動脈に誤つて刺入されて生じた脂肪栓塞例が散発的に報告されたが，眼科領域では幸にも報告に接することがなかつた。撚るに最近に及んで偶々油性ペニシリンの注射に際し起つた網膜中心動脈栓塞の一例に遭遇したのでここに報告する。

The 35 yr. old female has injected oil procain Penicillin to the right side of nasal base region by herself. After the injection of half dosis, she got blind of right eye suddenly accompanyingnausca and vomiting. Slight exophthalamus and swelling of eye lid were proved and ophthalamoscopic finding has shown severe embolia arteriae centralis retinae having oil in some of blood vesels. Vision could not be recovered and in third day, the oil Penicillin came out on the tip of nose. The author has considered this case as a kind of fat embolism. Experimentaly, the sme Penicillin was injected into the carotid artery of rabbit.
