簡易な新緑内障負荷試験—頸部圧迫時マ氏盲点拡大試験について（大橋教授開講15周年記念論文）

An available new loading test for glaucoma ; so-called angioscotometria under the jugular compression

Ⅰ．はじめに

　臨床上で賞用するにだる緑内障負荷試験の条件としては，（1）鋭敏で検出率が高く，早期診断に役だつこと，（2）安全で，かつ患者に苦痛を与えることが少ないこと，（3）簡易に施行しうること……などがあげられると思う。著者はすでに頸部圧迫時の眼圧上昇度が早期診断に役だつことを報告（日眼62巻，2347頁，昭33年）したが，今回はさらにこの頸部圧迫時にマリオット氏盲点が緑内障患者で早期から顕著な拡大をきたすことを認め，そのさいの葡萄膜・網膜血管の態度を検討し，上記の3条件を満たす新しい早期診断法として利用しうると考えたので，ここに大要を記し，大橋教授御開講15周年の記念にさせていただくことにした。

For the differential diagnosis and loading test of glaucoma, the author confirmed the dominant vertical axis increase of Mariotte's blind spot under the Jugular venous compression test by use of the author's Jugular compression band for 30 - 40mmHg. & campimetrical angioscotometrial by means of a red target in 3 - 5mm diameter.

A dominant increase of the blind spot as 15 degree over was shown in this test for glaucoma. The author considers that, most in glaucoma, an anoxial retinal disfunction should be mainly caused under the uveal stasis with the jugular compression.
