片眼にぶどう膜炎と重症筋無力症による眼瞼下垂をきたした1例

Unilateral blepharoptosis in a case of uveitis and myasthenia gravis

要約　目的：ぶどう膜炎の後，同眼に眼筋型の重症筋無力症（MG）を合併した症例の報告。症例：78歳女性が急激な左眼瞼下垂で受診した。発症の3年前に同眼に眼圧上昇を伴うぶどう膜炎の既往があり，治療のためにチモロールマレイン酸塩点眼液の使用歴があった。所見：テンシロンテストと抗アセチルコリン受容体結合抗体（抗AChR結合抗体）が陽性であり，眼筋型MGと診断された。パルス療法で眼瞼下垂は改善したが，抗AChR結合抗体値は上昇が続き，ぶどう膜炎は再燃を繰り返した。結論：自己免疫疾患であるMGは他疾患との合併が多く，ぶどう膜炎を併発することがある。眼圧治療の目的で使用されるチモロールマレイン酸塩点眼液は，MG発症と重症化の誘因となることがあり，注意を要する。

Abstract.　Purpose：To report a case of myasthenia gravis and uveitis with unilateral blepharoptosis. Case：A 78-year-old female presented with left blepharoptosis of acute onset. She had a history of uveitis with elevated intraocular pressure treated with topical timolol 3 years before. Findings：Corrected visual acuity was 0.7 right and 1.0 left. Ophthalmological findings, including ocular motility, were within normal limits. She was positive for tensilon test and antiacetylcholine, leading to the diagnosis of ocular myasthenia gravis. Blepharoptosis improved following pulsed corticosteroid therapy. The left eye showed recurrence of uveitis during the following 24 months. Conclusion：This case illustrates that myasthenia gravis may be associated with other diseases including uveitis. Past treatment with timolol may have triggered aggravation of myasthenia gravis.
