真菌性眼内炎に続発した脈絡膜新生血管に抗VEGF薬硝子体注射が奏効した1例

Intravitreal ranibisumab was effective for choroidal neovascularization secondary to fungal endophthalmitis

要約　目的：真菌性眼内炎に続発した脈絡膜新生血管にラニビズマブの硝子体注射が奏効した症例報告。症例：55歳女性が右眼の視力低下で受診した。5か月前に潰瘍性大腸炎の手術を受け，その1か月後に中心静脈カテーテル留置によると推定される真菌血症が発症し，抗真菌薬が投与されていた。所見：矯正視力は右0.8，左1.2で，両眼に黄白色斑が散在していた。右眼黄斑部に網膜剝離を伴う脈絡膜新生血管があった。右眼にラニビズマブの硝子体注射を行い，その1か月後に網膜剝離と脈絡膜新生血管は退縮し，1.2の視力を得た。1年後の現在まで再発はない。結論：真菌性眼内炎に続発した脈絡膜新生血管にラニビズマブの硝子体注射が奏効した。

Abstract.　Purpose：To report a case of fungal endophthalmitis with choroidal neovascularization（CNV）treated by intravitreal ranibizumab. Case：A 55-year-old female presented with blurring in the right eye. She had received surgery for ulcerative colitis 5 months before followed by hyperalimentation through central venous catheter. She developed infection by Candida albicans and was receiving antifungal drugs. Findings：Corrected visual acuity was 0.8 right and 1.2 left. Both eyes showed yellowish white patches in the retina. The right eye showed CNV with retinal detachment. The right eye received intravitreal injection of ranibizumab. CNV and retinal detachment subsided one month later with visual acuity improving to 1.2. There has been no recurrence for one year until present. Conclusion：Intravitreal ranibizumab was effective for CNV secondary to fungal endophthalmitis.
