Abstract. Purpose:To report a case with visual loss immediately after spinal surgery in the prone position. Case:A 49-year-old male underwent surgery for herniation of intervertebral disc in the prone position under general anesthesia lasting for 3 hours. The patient noticed loss of vision in the right eye after surgery. Findings:Visual acuity was no light perception right and 1.2 left. The right eye showed no light reflex,restricted movement in all directions,blepharoptosis and cherry-red fovea. Magnetic resonance imaging(MRI)showed anteriorly displaced right eye with the optic nerve forming a right angle to the eyeglobe. He was diagnosed with ischemic orbital compartment syndrome(IOCS). Ocular movements recovered 3 months later with no change in visual acuity. Conclusion:In the present case,increased intraorbital pressure appears to have induced impaired circulation in the orbit resulting in IOCS.
