Schwann細胞腫からの5-Sシステイニルドーパ産生が疑われた1例

A case of schwannoma with increased serum level of 5-S-cysteinyldopa

要約　目的：球結膜悪性黒色腫の切除後に血清の5-Sシステイニルドーパ値が上昇し，後縦隔Schwann細胞腫の切除後に血清値が正常化した症例の報告。症例：59歳女性に18年前からの左眼球結膜の色素斑が増大して受診した。所見と経過：左眼の角膜全周に色素沈着があり，角膜に侵入していた。生検で結膜悪性黒色腫と診断された。全身転移の所見はなく，1か月後に結膜腫瘍を切除した。血清の5-Sシステイニルドーパ値は正常範囲にあり，切除から14か月後に29.3nmol/lに上昇し，縦隔に腫瘍が発見された。右肺の部分切除と後縦隔腫瘍切除を行い，細気管支肺胞上皮癌と後縦隔Schwann細胞腫と診断された。血清の5-Sシステイニルドーパ値はその後正常化した。結論：本症例では，Schwann細胞腫から5-Sシステイニルドーパが産生された可能性がある。

Abstract.　Purpose：To report a case of malignant conjunctival melanoma who showed elevated level of 5-S-cysteinyldopa after resection that normalized after resection of mediastinal schwannoma. Case：A 59-year-old female presented with enlargement of pigment patch in her left eye that had appeared 18 years before. Findings：Her left eye showed pigmentation surrounding and invading the cornea. Biopsy led to the diagnosis of malignant melanoma. Clinical signs were absent suggestive of systemic metastasis. The melanoma was resected one month later. Serum level of 5-S-cysteinyldopa was within normal limits initially and increased to 29.3 nmol/l 14 months later followed by detection of mediastinal tumor. Resection of part of right lung and mediastinum led to the diagnosis of schwannoma and bronchioalveolar carcinoma of the lung. Level of 5-S-cysteinyldopa rapidly returned to normal value. Conclusion：The presenet case shows the possibility that 5-S-cysteinyldopa is produced from mediastinal schwannoma in the present case.
