Abstract. Purpose:To report a case of retinitis pigmentosa sine pigmento which was overlooked before cataract surgery. Case:A 67-year-old female presented with impaired visual acuity. She had had systemic hypertension,diabetes mellitus,and no night blindness. Findings:Visual acuity was 0.2 right and 0.1 left. Both eyes showed moderate cataract,pale optic disc,and central scotoma. Fluorescein angiography showed no macular edema. Magnetic resonance imaging(MRI)showed enhanced reflex suggesting optic neuropathy. Visual acuity decreased to 0.02 right and 0.15 left 8 years later apparently due to progression of cataract. Cataract surgery resulted in visual acuity of 0.05 right and 0.2 left with intense photophobia. Both eyes showed constricted visual field. Electroretinogram(ERG)was nonrecordable. She was diagnosed with retinitis pigmentosa sine pigmento. Conclusion:This case illustrates that ERG is essential in fundus disorders prior to cataract surgery.
