網膜中心静脈閉塞症に対するベバシズマブの硝子体内注射の短期成績

Short-term outcome of intravitreal injection of bevacizumab for central retinal vein occlusion

要約　目的：網膜中心静脈閉塞症に対するベバシズマブの硝子体内注射の短期成績の報告。対象と方法：網膜中心静脈閉塞症8例8眼を対象とした。男性5例，女性3例で，年齢は56～79歳（平均69歳）である。1眼が虚血型，7眼が非虚血型であった。発症から治療開始までの期間は2～19か月（平均8.8か月）であった。ベバシズマブ注射の回数は，1回が3眼，2回が4眼，4回が1眼であった。光干渉断層計で中心窩厚を測定し，視力はlogMARで評価した。結果：中心窩厚は注射1か月後で有意に減少した（p＜0.01）。0.2以上の視力変化を有意とするとき，2眼が改善，5眼が不変，1眼が悪化であった。結論：網膜中心静脈閉塞症に対するベバシズマブの硝子体内注射の短期観察で，黄斑浮腫は減少し，視力には明らかな効果はなかった。

Abstract.　Purpose：To report the outcome of intravitreal injection of bevacizumab for central retinal vein occlusion（CRVO）with macular edema. Cases and Method：This retrospective study was made on 8 eyes of 8 cases of CRVO. The series comprised 5 males and 3 females. The age ranged from 56 to 79 years,average 69 years. CRVO had been present for 2 to 19 months,average 8.8 months,at the time of treatment. Bevacizumab was injected once in 3 eyes,twice in 4 eyes,and 4 times in one eye. Foveal thickness was measured with an optical coherence tomography. Visual acuity was evaluated as logMAR. Results：Foveal thickness significantly decreased one month after treatment（p＜0.01）. Visual acuity improved by 0.2 or more in 2 eyes,remained unchanged in 5 eyes,and deteriorated by 0.2 or more in one eye. Conclusion：This short-time study shows that intravitreal injection of bevacizumab results in decrease of foveal thickness and no significant changes in visual acuity in CRVO.
