内境界膜剝離と重症例ではシリコーンオイル充填を併用した高度近視黄斑円孔網膜剝離の手術成績

Outcome of macular hole retinal detachment in high myopia after peeling of internal limiting membrane with or without silicone oil tamponade

要約　目的：高度近視黄斑円孔網膜剝離では，硝子体手術で内境界膜を剝離し，硝子体腔にC₃F₈ガスを充填しても再剝離する症例がある。これに対するシリコーンオイルタンポナーデの効果を検討した。症例と方法：過去46か月間に手術を行った黄斑円孔網膜剝離14例14眼を対象とした。年齢は55～88歳（平均66歳）であり，後部ぶどう腫が全例にあった。眼軸長は26.75～31.18mm（平均28.79mm）であった。硝子体切除の後，後部硝子体皮質ならびに内境界膜を剝離除去した。軽症例6眼にはC₃F₈，重症例8眼にはシリコーンオイルを硝子体腔に充填した。結果：14眼すべてで初回復位が得られた。結論：本症では，内境界膜を剝離したのち，シリコーンオイルタンポナーデを行うことで，高率に初回復位が得られる可能性がある。

Abstract.　Purpose：To evaluate the effect of silicone oil tamponade adjunct to peeling of internal limiting membrane（ILM）in the treatment of retinal detachment in high myopia with macular hole. Cases and Method：This retrospective study was made on 14 eyes of 14 patients of macular hole retinal detachment in the past 46 months. The age ranged from 55 to 88 years, average 66 years. All had posterior staphyloma. The axial length ranged from 26.75 to 31.18mm, average 28.79mm. After ILM peeling, tamponade with C₃F₈ was performed in 6 moderate cases and silicone oil in 8 severe cases. Results：Retinal reattachment was obtained in all the 14 eyes after initial surgery. Conclusion：Retinal reattachment can be facilitated by ILM peeling with adjunct tamponade by silicone oil in high myopic eyes.
