Purpose:To evaluate impaired ocular motility after surgery for aortic aneurysm or dissection. Cases:This study comprised 10 cases during the past 3 years. They received implantation of artificial vessel under extremely low body temperature,cessation of circulation,and cerebral perfusion. Results:All cases showed vertical and/or horizontal gaze palsy. Vestibulo-ocular reflex and Bell phenomenon were intact,suggesting supranuclear ocular motor disorders. Slow pursuit movement towards the paralyzed direction could be elicited in 6 cases. All the cases failed to show optokinetic nystagmus towards the direction of palsy. Five cases showed tonic deviation towards the direction of rotation of optokinetic nystagmus drum,suggesting impaired saccadic system. Diagnostic imaging showed lesions in the frontal lobe or its pathway to the brain stem in 3 cases only. Impairement of ocular motility recovered spontaneously. Conclusion:There is a high possibility that the obverved disorders of ocular motility are functional ones due to acute cerebral ischemia.
