Abstract. Purpose:To report cases of metastatic bacterial endophthalmitis treated by vitrectomy. Cases and Findings:Victrectomy was peformed on 5 eyes of 5 cases of metastatic bacterial endophthalmitis during the past 7 years 8 months. The ages ranged from 51 to 69 years,average 61 years. Interval between onset of ocular symptoms and vitrectomy in our hospital ranged from 2 to 77 days,average 21 days. Before surgery,endophthalmitis was diagnosed as metastatic in 1 case and idiopathic in 4. Ultimately,ophthalmitis was metastatic from liver abscess in 2 cases and from lung abscess,endocarditis and tuberculosis in one case each. The causative organisms comprised 2 cases of Klebsiella pneumoniae and one case each of Streptococcus anginosus,Mycobacterium tuberculosis,and unknown. Visual acuity before surgery ranged from nil to 0.01. Final visual acuity was nil in 3 eyes with retinal detachment before surgery. It improved in the other 2 eyes that had no retinal detachment. Culture of vitreous specimen was useful in establishing the diagnosis. Conclusion:Vitrectomy was useful in the treatment and diagnosis of metastatic bacterial endophthalmitis.
