　空気で水晶体囊を拡張させ，シリコーン眼内レンズ（IOL）をインジェクターで挿入する白内障手術を40眼に行った。通常の方法でヒーロンV^(R)を使ってIOLを挿入し，behind the lens（BTL）法で除去した20眼をBTL群，少量のヒーロンVと空気を使ってIOLを挿入し，rock'n roll（R＆R）法で除去した10眼をR＆R1群，同様に中等量のヒーロンVを使った10眼をR＆R2群とした。手術から24時間後のR＆R1群の眼圧は，他の2群よりも低値であった。手術から4時間後の眼圧が30mmHg以上の症例は，BTL群2眼，R＆R1群0眼，R＆R2群1眼であった。少量のヒーロンVと空気でIOLを挿入しR＆R法で除去する方法は，術後眼圧の上昇予防に有効な可能性がある。

　We performed cataract surgery on 40 eyes using 3 techniques after phacoemulsification-aspiration. In 20 eyes,silicone intraocular lens（IOL）was inserted using Healon V^(R)which was later removed by behind-the-lens procedure（BTL group）. In 10 eyes,IOL was inserted using small amount of Healon V and air. Healon V was removed by Rock'n Roll procedure（R＆R group 1）. In 10 eyes,IOL was inserted using moderate amount of Healon V and air. Healon V was removed by Rock'n Roll procedure（R＆R group 2）. Intraocular pressure（IOP）24 hours after surgery was significantly lower in R＆R group 1 than in the other two groups. IOP 4 hours after surgery was higher than 30mmHg in 2 eyes of BTL group,in none of R＆R group 1 and in one eye of R＆R group 2. Postoperative rise in IOP may be avoided by using small amount of Healon V and air during IOL insertion.
