眼瞼痙攣と片側顔面痙攣におけるボツリヌス毒素治療―治療間隔および再治療率からみた治療効果の検討

Treatment of blepharospasm and hemifacial spasm by botulinum toxin: Evaluation of the effect by recurrence and interval of treatment

　本態性眼瞼痙攣346例と片側顔面痙攣225例に対してA型ボツリヌス毒素の注射を繰り返して行った。注射の平均間隔は，眼瞼痙攣が4.7±1.7か月，顔面痙攣が6.1±2.1か月で，有意差があった（p＜0.0001）。治療の繰り返しによる効果の明らかな漸減はなかった。両群とも，約70％の症例が再治療を希望した。再治療を希望しない30％の症例での脱落理由は，治療の無効または症状の軽快で，後者はとくに顔面痙攣で多かった。以上の結果から，ボツリヌス毒素による治療は，本態性眼瞼痙攣と片側顔面痙攣に対して有効であると結論される。

We reviewed 571 patients who received injection of botulinum toxin A. The series comprised 346 cases of essential blepharospasm and 225 cases of hemifacial spasm. Interval of injection averaged 6.1±2.1months in cases of hemifacial spasm and 4.7±1.7months in cases of blepharospasm. The difference was significant（p＜0.0001,unpaired t-test）. There was no waning of effect after multiple treatments. In both groups,about 70％of patients desired to continue the treatment. The remaining 30％of patients dropped out because the treatment was ineffective or because of partial resolution which was more manifest in hemifacial spasm. The present findings show that botulinum toxin is effective for blepharospasm or hemifacial spasm.
