ボツリヌス療法は根治療法となりうるか

Can botulinum toxin become the definitive therapeutic modality?

　ボツリヌスA型毒素治療の効果を自験例371例について検討した。内訳は眼瞼痙攣153例と片側顔面痙攣218例である。平均年齢は65.6±11.6歳で，全例で1年以上の経過を追えた。複数回の治療により，眼瞼痙攣14例（9.2％）と顔面痙攣26例（11.9％）で，臨床症状が少なくとも1年間以上完全に消失した。これら奏効率は，従来報告されている自然寛解率よりも高く，本治療による効果であると判断された。症状が寛解した症例では，ほかの症例よりも，発症から短期間で本治療を開始していた。さらにこのような症例では，数回の治療で臨床症状が消失する可能性がある。

We reviewed 371 cases who were treated with botulinum toxin A. The series comprised 153 cases of blepharospasm and 218 cases of hemifacial spasm. Their ages averaged 65.6±11.6 years. All the cases could be followed up for 1 year or longer. Following multiple sessions of treatment,clinical manifestations disappeared for one year or longer in 14 cases（9.2％）of blepharospasm and in 26 cases（11.9％）of hemifacial spasm. These rates were higher than those of hitherto reported spontaneous remission,suggesting the efficacy of treatment with botulinum toxin A. Cases resulting in remission started the treatment earlier after onset than the rest. These cases may also show favorable response after lesser sessions of treatment.
