Purpose:To report a case of suprasellar meningioma masquerading as ischemic optic neuropathy at its onset. Case and Findings:A 48-year-old male presented with impaired vision in his left eye since 5 days before. His corrected visual acuity was 1.0 right and 0.1 left. Temporal visual field defect was present in the right eye and central scotoma in the left. Fluorescein angiography showed filling delay superior and inferior to the optic disc. The findings were suggestive of ischemic optic neuropathy. The visual acuity improved without systemic corticosteroid. He suffered severe headache 4 months later. Diagnostic imaging showed findings suggestive of suprasellar meningioma. Surgery led to the same diagnosis. Conclusion:This case illustrates that lesions near the sella turcica may simulate ischemic optic neuropathy at its onset. Diagnostic imaging was useful in the present case.
