A 29-year-old woman presented with temporal visual field defect in both eyes since 3 months before. Her visual acuity was 1.2 in either eye. Both eyes showed ring-shaped depigmentation around the optic disc of 2 to 3 disc diameters(DD)in width. Retinal arteries passing the depigmented area were more attenuated than distally. The blind spot was enlarged to twice the normal diameter. Optical coherence tomography(OCT)showed reduced thickness in the peripapillary retina. This area showed window defect by fluorescein angiography. Full-field electroretinogram(ERG)showed normal rod and cone responses. Multifocal ERG showed markedly reduced response in the atrophic peripapillary area. New lesions with focal visual field defect developed in her left eye during the follow-up. Eleven months after her initial visit,full-field ERG showed reduced cone response. The lesions appeared to reflect degenerated photoreceptors resulting in atrophy of retinal pigment epithelium and attenuated retinal arteries as is the case in retinitis pigmentosa. Above findings met the criteria of acute zonal occult outer retinopathy(AZOOR).
