蛍光眼底造影にて網膜静脈染を認めた網膜硝子体出血をきたした網膜動脈ループ症の1例

Vitreous hemorrhage in a case of retinal arterial loop formation manifesting dye staining of retinal veins by fluorescein angiography

目的：網膜硝子体出血で発見された網膜動脈ループ形成症に蛍光眼底造影で発見された網膜血管異常の症例報告。症例：29歳女性が5日前からの飛蚊症で受診した。視力は右1.2，左1.5であった。右眼に硝子体出血と網膜下出血があり，蛍光眼底造影で乳頭から耳上側に向かう動脈にループ形成，その末梢部の走行異常と途絶，これと交叉する網膜静脈の口径不同と色素染があった。結論：ループを形成した動脈が交叉部で静脈を圧迫して静脈血のうっ滞が生じ，先天性に屈曲，狭細化した動脈から出血が起こった可能性がある。

Purpose：To report a case of viteous hemorrhage with retinal vascular malformations. Case：A 29-year-old female presented with seeing floaters since 5 days before. Her visual acuity was 1.2 right and 1.5 left. Her right fundus showed vitreous and subretinal hemorrhage,and loop formation of superior temporal retinal artery. The more distal portion of the artery showed tortuosity and occlusion. Fluorescein angiography showed irregular caliber and dye staining of veins at the site of arteriovenous crossing. Conclusion：There is a possibility that the looped retinal artery induced venous stagnation by pressure at the site of arteriovenous crossing. Vitreoretinal hemorrhage may have been due to congenital tortuosity and narrowing of distal portion of retinal artery.
