　抄録：変性辷り症，変性側彎症，分離辷り症などの腰椎変性疾患や後側方固定後偽関節など42例の腰椎・腰仙椎の多椎間固定に，Isola spinal implant systemのtranspedicular screw法を採用した．うち28例は仙椎を含めた固定であった．2椎間固定が29例，3椎間以上の固定が13例であった．脊椎腫瘍の1例に前方から，破壊性脊椎関節症1例と破裂骨折1例の計2例に後方から椎体置換したが，その他は後側方固定のみであった．臨床症状の改善は良好であった．側彎変形の矯正は良好で，腰椎の生理的前彎も獲得された．骨癒合率は95％で，2例の偽関節は，いずれも腰仙椎固定例のL5/S1椎間であった．手術による神経根障害の発現や悪化はなかった．スクリュー／ロッド間の連結に自由度が大きいIsola spinal systemは，側彎や回旋変形を伴う腰椎・腰仙椎の多椎間固定においても，安全・確実なsegmental fixationを可能にした．
