抄録：腰椎変性辷り症に対する内側椎間関節切除術とcombined distraction and compression rod system併用の後側方固定術を施行して，術後3年から10年経過した180例を調査した．除圧は全椎弓切除は必要なく，内側椎間関節切除術によるlateral recessの解放とcentral canal stenosisには肥厚黄色靱帯や椎弓腹側の切除で十分な神経症状の改善が得られた．同時に後側方固定術を行うことは，辷り部の椎間不安定性や辷りの進行を阻止し，臨床症状の改善をより確実なものとした．隣接椎間の変性は9％で観察されたか，全例て長期成績で満足すべき臨床症状と神経学的改善が得られていた．combined systemは，早期離床にもかかわらず高い骨癒合率（97％）の獲得と，生理的前彎位の保持に有用で，簡便で安全なinstrumentationであった．
