骨粗鬆症性椎体骨折に対する早期診断・保存的治療の重要性について

Importance of Early Diagnosis and Conservative Treatment of Osteoporotic Vertebral Fractures

　背景：骨粗鬆症性椎体骨折に対する早期保存的治療の重要性を確認する．

　対象と方法：骨粗鬆症性椎体骨折患者に施行した軟性コルセット群と体幹ギプス群において，発症後7日以内に治療を開始した症例を，各群でそれ以後に治療を開始した症例とで比較検討した．

　結果：軟性コルセットで治療した安定型は，治療開始時期に関係なく骨癒合率は100％であったが，体幹ギプスで治療した不安定型の骨癒合率は，発症後7日以内に治療開始すれば99％で，それ以後に治療を開始した症例の93.4％より有意に良好な結果であった．

　まとめ：不安定型骨折に対する早期診断・保存的治療の重要性が示された．

　Background：To confirm the importance of early conservative treatment of osteoporotic vertebral fractures under hospitalization.

　Methods：We compared cases treated within 7 days with cases treated 8 days or more after onset, in a Corset Treatment Group and a Body Cast Treatment Group, respectively.

　Outcome：The bone union rate of stable fractures was 100% in the Corset Treatment Group regardless of when the conservative treatment was started.　The bone union rate of the unstable fractures treated within 7 days in the Body Cast Treatment Group was 99%, and significantly better than the 93.4% in the group treated after 8 days or more.

　Conclusion：The results suggested that early diagnosis and conservative treatment is important for unstable osteoporotic vertebral fractures.
