下顎骨縦割アプローチにより全摘した軸椎骨巨細胞腫の1例

Mandible-splitting Approach for Giant-cell Tumor of Axis : A Case Report

　軸椎に発生した骨巨細胞腫(GCT)に対して，下顎骨縦割アプローチによる全摘術を行い，良好な結果を得たので報告する．症例は23歳の女性で，1年来の頚部痛を主訴に近医を受診し，X線，MRIで軸椎腫瘍を指摘され当科に入院となった．経口的生検術による病理組織検査でGCTと診断された．術前に腫瘍塞栓術を施行し，後方から腫瘍摘出術と固定術を施行した．2期的に下顎骨縦割アプローチにより腫瘍摘出術と自家骨移植を行った．術後に移植骨が前方へ脱転を来したため，移植骨を除去し後方に骨移植を追加した．強固な骨癒合が得られ，術後2年の段階で腫瘍の再発を認めていない．

　We report a rare case of giant-cell tumor of axis.　A 23-year-old woman presented with severe pain in the neck that persisted for a year.　Radiographs and computed tomography (CT) scans showed tumors involving and destroying the body and arch of the axis.

　A pathological specimen obtained by transoral biopsy revealed giant-cell tumor with multinuclear giant cells and mononuclear stromal cells.　After embolization of the feeding arteries, the tumor was totally resected through combined anterior mandible-splitting approach followed by bone grafting through the posterior approach.　However, 3 months after surgery, the graft dislodged, which required graft retrieval and posterior fusion reinforcement with an additional bone graft.　At the 2-year follow-up, complete fusion without tumor recurrence was achieved.
