　稀な疾患であり肺などに転移する良性転移性平滑筋腫(benign metastasizing leiomyoma：BML)が脊椎に転移した1例を経験したので報告する．症例は子宮筋腫摘出術の既往のある47歳の女性で，主訴は腰下肢痛であった．MRIではL3高位で脊柱管内外に充実性腫瘍を認めた．腫瘍部分摘出術を施行したところ，摘出標本は子宮筋腫の手術標本と類似した組織像を認めたためBMLと診断した．またエストロゲン受容体，プロゲステロン受容体ともに陽性であったため，残存腫瘍に対しホルモン療法を行い，残存腫瘍の縮小を認めた．

　Benign metastasizing leiomyoma (BML) is a benign tumor that develops in patients with a past history of uterine myoma.　Although BML is recognized as a benign tumor, metastasis to other organs, including the lungs, has been reported.　We report a case of BML that metastasized to the lumbar spine.　A 47-year-old female with a history of hysterectomy for a benign leiomyoma developed severe pain in the low back area and lower extremities.　Magnetic resonance imaging revealed a tumor in the lumbar region.　The tumor was resected and diagnosed as BML by histological examination of the surgical specimen.　Since immunohistological staining for both estrogen and progesterone receptors was positive, hormone therapy with LH-RH was performed postoperatively, and it resulted in reduction of the size of the residual tumor.
