小児における膝蓋骨剥離骨折の治療経験

Avulsion Fractures of the Upper and Lower Pole of the Patella in Children

　小児に発生する稀な骨折である膝蓋骨の剥離骨折を4例経験した．年齢は10～15歳で，すべて男性であった．いずれも観血的治療を行い，下極骨折の3例はKirschner鋼線(K-wire)や軟鋼線による骨接合術を行い，上極骨折の1例は骨片ごと大腿四頭筋腱を膝蓋骨に縫着した．すべて骨癒合が得られ，可動域も良好であった．膝伸展機構損傷である本骨折は，たとえ転位のわずかな場合でも，観血的治療によって正確な整復と強固な内固定行うべきであり，骨片の大きさや形状に応じて，最も適した内固定方法を選択するのがよいと考える．

　Fractures of the patella are rare in children.　We report 4 cases of avulsion fractures of the patella in children, including 3 cases of lower pole and 1 case of upper pole fractures.　All 4 cases were treated by open reduction and internal fixation, and bone union was achieved with no impairment of function.　Anatomical reduction and rigid internal fixation are necessary to repair extensor mechanism and to regain normal function of the knee.
