脛骨遠位骨端線早期閉鎖に対しLangenskiöld手術を行った1例―術後10年間の経過追跡

Surgical Treatment of Early Partial Closure of Distal Tibial Epiphysis by Langenskiöld Operation. Case Report with 10-years Follow-up Observation

　症例は6歳の男児．交通事故で受傷し，左足関節の内反変形を主訴に受診し，左脛骨遠位骨端線離開後の骨端線早期閉鎖を認めた．骨性架橋切除ならびに遊離脂肪移植からなるLangenskiöld手術を行い，術後10年間の経過追跡で良好な結果を確認できた．Langenskiöld手術は自然矯正が期待できる，侵襲の少ない効果的な手術方法である．早期骨端線閉鎖に対する早期手術として，Langenskiöld手術は有用な選択肢であると考えられた．

　Post-traumatic premature closure of growth plate in long bone is not a rare entity, and sometimes results in various degree of deformity and instability.　We present a case report of early partial closure of distal tibial epiphysis of a 6-year-old boy, treated by Langenskiöld operation consisting of bony bridge resection and free fat grafting.　Ten years continual follow-up observation revealed a satisfactory clinical outcome with good skeletal remodeling and no recurrence of deformity.　The results suggest the effectiveness of the Langenskiöld procedure to prevent the recurrence of postoperative deformity, and restore the longituidinal growth of limb.
