尺骨遠位骨端線早期閉鎖による手関節部変形に対して橈骨楔状短縮骨切り術を施行した1例

Wedge-shortening Osteotomy of the Radius for Wrist Deformity as a Result of Distal Ulnar Epiphyseal Arrest. Case Report

　比較的稀な尺骨遠位骨端線損傷による尺骨成長障害および橈骨遠位端の二次性変形によって，手関節痛と前腕・手関節可動域制限を来した症例に対して橈骨楔状短縮骨切り術を施行し，良好な成績を得たので報告する．橈骨遠位端の二次性変形は臨床症状の原因となり，手術適応・術式選択を左右する重要な因子となる．

　Epiphyseal injury of the distal ulna is relatively rare.　Growth arrest and the resulting shortening of the ulna often causes secondary deformity of the distal radius characterized by increased radial inclination and a convex sigmoid notch.　A 16-year-old boy with growth arrest of the ulna complained of wrist pain and restricted pronation and radial deviation of the wrist.　Secondary deformity of the distal radius appeared to be the cause of his symptoms.　Since after a joint-leveling procedure, the convex sigmoid notch might eventually cause osteoarthritic change in the distal radioulnar joint, he was treated by wedge-shortening osteotomy of the radius.　The postoperative course was favorable.
