 Epiphyseal injury of the distal ulna is relatively rare. Growth arrest and the resulting shortening of the ulna often causes secondary deformity of the distal radius characterized by increased radial inclination and a convex sigmoid notch. A 16-year-old boy with growth arrest of the ulna complained of wrist pain and restricted pronation and radial deviation of the wrist. Secondary deformity of the distal radius appeared to be the cause of his symptoms. Since after a joint-leveling procedure, the convex sigmoid notch might eventually cause osteoarthritic change in the distal radioulnar joint, he was treated by wedge-shortening osteotomy of the radius. The postoperative course was favorable.
