 We report a rare case of pisotriquetral joint loose body that gave rise to ulnar wrist pain. A 49-year-old man experienced left ulnar wrist pain when he struck the ground with his golf club while swinging his golf club. Pain occurred during ulnar deviation of the wrist and pronation of the forearm. Plain radiographs of the wrist demonstrated a small bone fragment in the distal ulnocarpal space, and a pisotriquetral loose body was removed by open pisotriquetral joint capsulotomy. Excision of the loose body provided complete relief from the pain, and the patient returned to playing golf. Pisotriquetral loose body should be included in the differential diagnosis of patients with ulnar wrist pain.
