ゴルフ愛好家に発生した豆状三角関節内遊離体の1例

Pisotriquetral Joint Loose Body Incidentally Occurred in an Amateur Golfer. Case Report

　豆状三角関節内に発生した遊離体により疼痛を生じた稀な症例を経験した．症例は49歳の男性であった．ゴルフのスイングの時に地面を叩いてから左手関節尺側に疼痛と前腕回内時の引っかかり感が出現した．左手関節尺屈と前腕回内動作で疼痛が誘発され，単純X線像では左尺骨手根間隙遠位に骨片を認めた．手関節部尺側皮切による豆状・三角関節の切開により，遊離体が摘出された．遊離体摘出により引っかかり感と痛みが消失し，1カ月でゴルフを再開した．手関節尺掌側部痛を生じる疾患として豆状三角関節内遊離体も鑑別に置く必要がある．

　We report a rare case of pisotriquetral joint loose body that gave rise to ulnar wrist pain.　A 49-year-old man experienced left ulnar wrist pain when he struck the ground with his golf club while swinging his golf club.　Pain occurred during ulnar deviation of the wrist and pronation of the forearm.　Plain radiographs of the wrist demonstrated a small bone fragment in the distal ulnocarpal space, and a pisotriquetral loose body was removed by open pisotriquetral joint capsulotomy.　Excision of the loose body provided complete relief from the pain, and the patient returned to playing golf.　Pisotriquetral loose body should be included in the differential diagnosis of patients with ulnar wrist pain.
