　症例は，L5/S1左外側狭窄によるL5神経根症の70歳の女性である．腰椎単純X線像で多椎間に真空現象を呈し，MRIではL5/S1左側椎間孔入口部から外側にT1，T2強調画像低輝度の円形の陰影を認めた．ミエロ後CTでも同部位に椎間板内ガス像と同輝度の陰影を示していたことから，脊柱管内および外側にガス像を伴った椎間孔狭窄症として，L5/S1 monoportal PLIF(後方椎体間固定)を施行した．術後左下肢痛は消失した．

　A 70-year-old woman had a 4-year history of pain in the left buttock and lateral leg.　The patient had been diagnosed with left L5 radiculopathy secondary to L5-S1 lateral stenosis.　She had been treated conservatively, and her symptoms had improved.　After a fall in May 2008, the left leg pain became severe.　Plain lumbosacral radiographs showed narrowed intervertebral disc spaces and intradiscal gas (vacuum phenomenon) from L2-3 to L5-S1.　MRI T1- and T2-weighted images revealed a low, rounded lesion at the entrance of the left L5-S1 foramen and extraforamen.　Computed tomographic myelography revealed gas collection inside and outside the spinal canal at L5-S1.　Monoportal PLIF at L5-S1 resulted in complete pain relief.
