手根骨尺側長軸脱臼の1例 Traumatic Axial-ulnar Dislocations of the Carpus. 　手の圧挫による手根骨尺側長軸脱臼の1例を経験した．症例は，18歳の男性である．作業中に右手をプレス機にはさみ受傷した．手指は中指-環指間で離開し，環指と小指は回内位をとっていた．明らかな神経脱落所見は認められなかった．画像所見では，手根骨は有頭骨-有鉤骨間で離開しており，有鉤骨が掌側へ回内転位していた．手根骨尺側長軸脱臼と診断して，有頭骨-有鉤骨固定術を施行した．術後経過は良好で，術後3カ月で現職に復帰した．本損傷は極めて稀な外傷であるが，良好な機能的予後を獲得するためには早期の適切な診断と治療が重要である．

　We reported a case of eighteen-year-old male with traumatic axial-ulnar dislocation of the carpus.　He injured his right hand by press machine.　The 3th interdigital area was spread with rotational deformity of ring and little finger.　Plain X-ray and CT demonstrated the hamate dislocation to ulnar direction with 4th and 5th metacarpal bone.　Open reduction and capitate-hamate fixation was performed using the DTJ screw and several pins.　He could return to his previous work with no limitation 3 month after surgery.　The axial dislocation of the carpus is rare injury of the hand.　Early management of both the skeletal and soft tissue components of the injury seems most important for good outcome.
