橈骨頭骨折後の軟骨欠損に対し骨軟骨柱移植を行った1例

Treatment of Cartilage Defect after Radial Head Fracture by Osseocartilage Column Plantation. Case Report

　今回われわれは橈骨頭骨折後の軟骨欠損に対し骨軟骨柱移植を行い，良好な結果が得られたので報告する．症例は62歳の女性で，自転車に乗っている時に転倒し受傷した．当院整形外科を翌日受診し，X線像で右橈骨頭骨折を認めた．またCTにおいて関節面の陥没を認めた．4週間シーネ固定とした後，リハビリテーションを開始したが，著明な右肘関節可動域制限と疼痛を認めた．受傷後7カ月目に膝関節から骨軟骨柱を採取し，欠損部に移植を行った．その後，関節可動域制限および疼痛は消失した．

　We treated a cartilage defect after a radial head fracture by osseocartilaginous column implantation.　The patient was a 62- year old woman who injured her right elbow in a fall from her bicycle and came to our hospital the next day.　Radiographs showed a radial head fracture.　Rehabilitation was started after external fixation of the fracture for four weeks, but a markedly limited range of motion of her right elbow and pain were noted.　Seven months later, we treated a cartilage defect in the radial head by osseocartilagious column plantation.　After the operation, she recovered almost full range of motion of her elbow.
