 We report two cases of C5 palsy following microendoscopic decompressive laminotomy (MEL) for treating cervical spondylotic myelopathy. Patients were 2 men aged-76 and 63. Both had C5 palsy on the approach side and multilevel decompression. Paralysis developed after 2 days and improved with conservative therapy. The possible mechanism leading to C5 palsy after MEL to multilevel involvement is the posteromedial shift of the dural sac when interosseous segments are removed.
