これまで筆者らは転移性脊椎腫瘍の後方除圧固定術として,G-rodを用いたsegmental spinal instrumentation(G-rod SSI法)を行いその有用性を報告してきた.本研究ではG-rod SSI法のsublaminar wireとしてチタンケーブルと超高分子量ポリエチレンケーブル(UHMWPEケーブル)を用いた強度比較試験を行った.繰り返し試験,破壊試験ともに,UHMWPEケーブルによる固定法はチタンケーブルによる固定法と同等の固定力を持ち,高い弾性力を持ち合わせていた.UHMWPEケーブルを用いたG-rod SSI法は固定隣接椎間へのストレスを軽減させうる方法と考えられた.
We have reported clinical results of G-rod SSI with titanium cables as an instrumentation for metastatic spinal tumors. To clarify utility of the ultra high molecular weight polyethylene cable (UHMWPE cable) for the G-rod SSI, we performed a biomechanical study using injured spinal models instrumented by the G-rod SSI. The spine with G-rod was combined either titanium cables or UHMWPE cables. Elasticity of the instrumentation with UHMWPE cables was higher than that with titanium cables although strength was almost equal. The spinal instrumentation by G-rod SSI with UHMWPE cables may decrease delayed adjacent intervertebral problems for osteoporotic patients with metastatic spinal tumors.
