 Freiberg infraction is aseptic necrosis of the head of the second metatarsal bone and is most common in teenage women. We report a rare case in a 77-year old woman. It was difficult to make the diagnosis, because there were no abnormal radiographic findings on her first visit. Magnetic resonance imaging was very useful in ultimately making the diagnosis. Osteochondral plugging of the head of the bone was performed and good results had been obtained 8 months after surgery.
