　血管平滑筋腫は中年女性の下肢に好発する．手における血管平滑筋腫は比較的稀であり，また血管平滑筋腫は有痛性であるために2cm以下で摘出術を施行されることが多い．われわれは非常に稀な手掌発生で腫瘍径が5cmの巨大な血管平滑筋腫を経験した．症例は72歳，男性で20年以上前に腫瘤に気づいたが，疼痛なく放置し，徐々に増大した．腫瘍は単純摘出術を施行され，病理組織診断にて血管平滑筋腫と診断された．

　A vascular leiomyoma typically occurs in the lower extremities of middle-aged women. A vascular leiomyoma in the hand was relatively rare and most of that were removed under 2cm in size due to the characteristic pain of the nodules. We report a 72-year-old man who presented with a slowly growing mass for approximately 20 years in the palm which, on histopathological findings, was shown to be a large (the widest diameter 5 cm) vascular leiomyoma.
