症例は12歳の男児である.自転車で走行中に転倒し,左環指の2度のデグロービング損傷を認めた.近医にて処置されたが,指全体がうっ血状となり受傷後3日目に当科を紹介された.指尖部のみを残して剝脱した皮膚を切除し,上腕外側皮弁と筋膜皮弁を用いて欠損部を被覆し血行再建を行った.再建指は機能的にも整容的にも満足が得られ,上腕外側皮弁は指の高度な複合組織損傷に対する治療の選択肢の1つになりうると考えた.
We report a 12-year-old boy whose ring finger sustained a classⅡdegloving injury when he was run over by a bicycle. The finger was treated in a nearby clinic but then congestion gradually occurred, and he was referred to our hospital three days after the injury. The degloved skin was resected and the exposed tissue was covered using a lateral arm free flap and a fascial free flap. The reconstructed finger had satisfactorily restored function with a good cosmetic result. The authors believe the lateral arm free flap can be a useful method of finger reconstruction following severe complex tissue injuries.
