 A 67-year-old woman with spondylolisthesis of the fourth and the fifth lumbar vertebrae underwent posterior lumbar interbody fusion at another hospital for disabling pain in her lower back and both legs. On the initial visit to our clinic she complained of no improvement in her symptoms and was found to have pseudarthrosis and progression of the slip at the L5-S1 level. Posterior stabilization with trans-sacral interbody fusion was performed as salvage therapy, and solid fusion was achieved with relief of her symptoms. Trans-sacral interbody fusion proved to be effective for the treatment of severe spondylolisthesis.
