Three cases of sacral stress fracture in semi-professional long-distance runners are reported. A 20-year-old male and 21 year-old female started running again after restriction of training for two months, and a 21-year-old female with amenorrhea and low bone density started running 5 months after her initial visit. There were no long-distance runners but the three cases who were treated for sacroiliac pain. A stress fracture of the sacrum should be suspected in long-distance runners who complain of low back pain, especially sacroiliac pain. MRI is very useful in making the diagnosis of this rare injury.
