両側アキレス腱同時皮下断裂1例の治療経験

Simultaneous Bilateral Rupture of the Achilles Tendon : Case Report

　抄録：両側のアキレス腱が同時に断裂することは稀であり，本邦での報告も22例を数えるにすぎない．今回われわれは全力疾走中，ほぼ同時に両側のアキレス腱を断裂した症例を治療する機会を得たので報告する．症例は65歳の男性，刃物を持って向かってきた不審者から全力で走って逃げていた際に受傷した．両アキレス腱部に陥凹を認め，Thompson兆候は陽性であった．両側同時断裂と診断し，本症例に対し早期の社会復帰を目的に手術療法を選択した．手術は内側縦切開で進入し，2号ファイバーワイヤー（Arthrex社製）を用いてPennington法にて腱縫合を行った．この縫合糸は従来のポリエステル糸の約2倍の強度と耐磨耗性を持つとされ，術後の早期運動療法に有用と考えた．術後3週で下腿ギプスを除去，足関節の可動域訓練を開始し，4週で平行棒内での歩行訓練を行った．5週でTステッキ歩行が可能となり良好な経過を得た．

　We report a case of simultaneous bilateral rupture of the Achilles tendon sustained while running at full speed. This injury is rare, and there are only 22 reports in the Japanese literature. We treated the ruptures surgically by repairing them with the recently developed No. 2 nonabsorbable multifilament Pennington sutures. Postoperatively, immobilization was achieved with bilateral below-knee casts in the equinus position for 3 weeks, after which range of motion and strengthening exercises were begun. At 4 weeks the patient regained full range of motion. Almost full weight bearing in a walker was started at 5 weeks, and at 6 weeks the patient became able to walk with a T-cane.
