頚椎症に対する後方除圧＋後側方固定術（桐田-宮崎法）の10年以上の長期成績

Long-term Outcome of Posterior Decompression with Posterolateral Fusion (Kirita-Miyazaki Procedure) for Cervical Spondylosis

　抄録：今までに報告のない頚椎症に対する後方除圧＋後側方固定術（桐田-宮崎法）の10年以上の長期術後成績を調査可能であった28例について臨床的，X線的に検討した．臨床症状は，術後24例（86％）が改善され長期的にも安定していた．しかし，一過性だが術後C5，C6神経根障害が2例（7％）にみられた．X線的には術後後弯変形が16例（57％）にみられ，術後悪化の2例はともに後弯変形を呈していた．不安定性は1例のみだが症状は改善していた．骨癒合は終診時でも7例（25％）しかみられなかった．可動性は減少しつづけ終診時71％減少していた．以上より本法は長期にわたり良好な術後成績を得ることが可能で，骨癒合は期待されないが可動域の減少により不安定性がなく良好な術式と思われた．しかし，術後の一過性のC5，C6神経根障害や，術後後弯変形が57％にみられたことより，さらに術式の工夫が必要と思われた．

　The long-term outcome of the posterior decompression with posterolateral fusion procedure developed by our department for cervical spondylosis has never been reported. In this study the long-term outcome of 28 patients was evaluated clinically and radiologically. Although it restricts the range of motion, posterior decompression with posterolateral fusion prevents instability and the long-term clinical outcome is generally good. However, since kyphotic deformity developed postoperatively in more than half of the patients and transient postoperative radiculopathy of the C5 and C6 nerve roots occurred in two patients, the method may need to be modified to achieve better results.
