 We report a case of bilateral insufficiency fractures of the medial tibial plateau in a 66-year-old female who had been treated for rheumatoid arthritis. The patient complained of severe pain in both knees for the past month. A plain radiograph revealed a transverse fracture line in the medial tibial plateau of the left knee, but no fracture line was visible in the right knee, and we diagnosed the fracture of the riget knee based on the MRI images, which showed changes in intensity at the fracture site in the right knee. We treated the patient conservatively with lateral wedge insoles and a walking aid, and the outcome was satisfactory. Insufficiency fractures should be borne in mind when examining the knee joints of patients such as rheumatoid arthritis.
