大腿骨骨幹部骨折を来したpycnodysostosisの1例

Pycnodysostosis with Femoral Shaft Fracture : Case Report

　抄録：症例は76歳，女性．2001年6月下旬に特に誘因なく左大腿部痛が出現し，前医を受診するもX線上異常はないといわれ歩行していた．7月10日に再び左大腿部痛が出現し，歩行困難となり当院を紹介され受診した．小学校1年生のときから15～16回の骨折の既往がある．両親はいとこ結婚である．同胞には異常を認めない．身体所見としては，小人症を呈し特徴的な鳥様顔貌を認め，指趾は短縮し爪は変形している．X線像上，泉門は開存し下顎角の消失を認め，手指末節骨は溶骨性骨欠損を呈する．血液学的所見には異常を認めない．本症例はpycnodysostosisに生じた左大腿骨骨幹部骨折であると診断し，7月17日に上腕骨ネイルを用いた閉鎖性髄内釘固定術を施行し，術後7週より超音波骨折治療を併用した．術後1年の現在，良好な骨癒合が得られている．

　A case of pycnodysostosis with closed femoral-shaft fracture in a 76-year-old woman is reported. The patient had bone abnormalities manifested by short stature, hypoplasia of the mandible, open sutures, and hypoplasia of the distal phalanges of the fingers and toes, she had experienced frequent spontaneous fractures of the extremities. Her recent pathologic femoral-shaft fracture was treated with a closed intramedullary nail, followed by ultrasound therapy. Treatment was successful, and solid bone union was observed on a radiograph 8months after surgery.
