発熱，高度貧血，肝機能障害などの全身症状を呈した通常型悪性線維性組織球腫の1例

The Ordinary Type of Malignant Fibrous Histiocytoma Presenting with Constitutional Symptoms Such as Pyrexia, Deep Anemia and Liver Dysfunction. Case Report

　発熱などの全身症状を呈した29歳の男性で，右大腿の悪性線維性組織球腫（MFH）の1例を報告する．発熱，白血球増加，輸血を要する高度貧血，肝機能障害などの症状で内科的精査後に当院を紹介された．大腿部に20cm大の腫瘤を認め，針生検で，軟部肉腫と診断，股関節離断術を施行した．術後通常型のMFHと最終診断した．術後早期から全身症状は改善した．術前血中G-CSF，IL-6が高値であったが，術後は正常値となり，これらのサイトカイン産生腫瘍と判断した．現在，手術後8年であるが，無病生存中である．

　We reported a case of malignant fibrous histiocytoma (MFH) presenting with constitutional symptom such as pyrexia. A 29-years-old man with a large tumor of his right thigh was referred to our hospital after a close inspection by his internist for high fever, leukocytosis and severe anemia. The tumor was diagnosed as a sarcoma by needle biopsy. Hip disarticulation was performed. The excised tumor was diagnosed as the ordinary type of MFH. After the surgery the patient's symptos promptly improved within two weeks. The serum level of G-CSF and IL-6 were very high before surgery and normalized after surgery. The tumor was believed to have been responsible for the production of these cytokines. The patient has survived disease free for eight years after surgery.
