純粋語聾を呈したテント髄膜腫の1例

A Case of Tentorial Meningioma Presented with Pure Word Deafness

　純粋語聾は聴放線ないし側頭葉病変によるWernicke領域への両側の聴覚入力遮断によって起こり，脳血管障害に起因することが多い。今回われわれは，純粋語聾を呈したテント髄膜腫の1例を報告する。症例は73歳の女性。テント髄膜腫による閉塞性水頭症で発症し脳室腹腔短絡術を受け，1年後に聴力障害を呈し入院した。入院時，内言語に障害はないが聴理解および復唱が著しく障害され，純音聴力はほぼ正常，聴性脳幹反応はI-V波は正常でVI波以降が消失，中間潜時反応ではpeakを認めなかった。MRIでは髄膜腫による両側内側膝状体の圧迫を認め，聴放線／側頭葉皮質には異常を認めなかった。^99mTc-HMPAO SPECTでは左側頭葉の血流低下を認め，聴覚入力遮断によるdiaschisisと考えられた。本症例では，髄膜腫による両側内側膝状体圧迫により左側頭葉への聴覚入力が遮断され，純粋語聾を呈したものと考えられた。

It has been known that an isolation of Wernicke's area from auditory input results in pure word deaf-ness. In this report, a 73-year-old female case with tentorial meningioma suffering from pure word deaf-ness is reported. The patient initially presented with hydrocephalus, and was treated with a ventriculo-peri-toneal (V-P) shunt. A year after the V-P shunt, she suffered from a symptom of deafness. On admission, her repetition and auditory comprehension were se-verely impaired, while reading and visual comprehen-sion were almost normal.
